Job Summary The Foster Care Specialist is responsible for providing essential support and services to children and families within Embracing Heart's placement program. Under the direct supervision of the agency Director, you will ensure the well-being and progress of children in placement, facilitate effective communication between all parties involved, and maintain accurate documentation. This role requires a compassionate individual with a strong understanding of social work principles and a commitment to providing high-quality care.
Responsibilities
●Caseload Management: ■Manage and monitor assigned caseloads, ensuring the needs of each child are met. ■Conduct regular assessments of children's needs and develop appropriate service plans. ■Coordinate and facilitate communication with children, foster families, biological families, and other service providers.
●Documentation and Reporting:
■Maintain accurate and up-to-date case files, including progress notes, service plans, and incident reports. ■Prepare and submit required documentation and reports to the Placement Supervisor in a timely manner. ■Ensure all documentation complies with Nebraska state statutes and regulations.
●Service Delivery: ■Provide direct support and advocacy to children in placement, addressing their emotional, social, and developmental needs. ■Facilitate visits and interactions between children and their families, as appropriate. ■Assist in the development and implementation of individualized service plans. ■Work to ensure that identified child needs are met, and that services are progressing as planned.
●Supervision and Collaboration: ■Participate in weekly supervision meetings with the Placement Supervisor to review caseloads, discuss progress, and address concerns. ■Submit work, projects, and identified materials as directed by the Placement Supervisor. ■Collaborate with other members of the Embracing Heart team to ensure coordinated service delivery.
●Compliance and Professional Development: ■Adhere to all Embracing Heart policies and procedures, as well as Nebraska state statutes and regulations. ■Participate in ongoing professional development and training as required. ■Maintain confidentiality of client information.
●Performance Evaluation: ■Participate in annual performance evaluations with the placement supervisor.
Requirements: ●Bachelor's degree in Social Work, Counseling, or a related human service field. ●Verification of educational qualifications through submission of official transcripts, degrees, diplomas, and certificates. ●Strong understanding of child welfare principles and practices. ●Excellent communication, interpersonal, and organizational skills. ●Ability to work independently and as part of a team. ●Proficiency in documentation and report writing. ●Commitment to providing culturally competent and trauma-informed care. ●Ability to pass all required background checks. ●Must possess a valid driver's license and reliable transportation. ●Must be able to work flexible hours as needed.
