As an LPN with Embrace Home Care Agency LLC, your role will be vital in ensuring our clients receive the care they need to live comfortably and independently. Your key responsibilities will include:
- Providing direct patient care in accordance with care plans.
- Monitoring and documenting patient health, including vital signs and changes in condition.
- Administering medications and treatments as prescribed.
- Collaborating with other healthcare professionals to ensure the best outcomes for clients.
- Educating clients and their families about care plans and health management.

We’re seeking a compassionate and skilled LPN who meets the following qualifications:
- Active LPN license in the state of South Carolina.
- At least 1 year of experience in a healthcare setting.
- Strong communication and interpersonal skills.
- A commitment to providing high-quality, patient-centered care.
- The ability to work independently and as part of a team.
