Residential Case Manager PRTF

To be a successful Residential Case Manager you must be willing and become able to do the following always:
1. Coordinate the overall care for clients in conjunction with the therapist and develop an individual plan to successfully exit to a community setting using a trauma informed approach focused on resilience and recovery.
2. Assist in the selection of clients for the facility and complete intake paperwork with youth upon admission.
3. Communicate with external stakeholders regarding individualized plan progress.
4. Achieve positive discharge outcomes.
5. Follow Company Policy and Procedure and operate according to the Employee Handbook and Residential Case
Manager Guide
6. Achieve stakeholder satisfaction goal.

To qualify for this job, you need to have:

• Be at least 21 years of age.
• HS Diploma or GED with 3 years of experience in either case management or with youth in a facility, OR a Bachelor’s degree is required; BSRB license preferred
• The ability to write and use a computer.
• Ability to travel, driving a company or personal vehicle including at night.
• Physical Requirements: Ability to walk, including up stairs, and stand for extended periods of time. Ability to complete an emergency safety intervention as trained. Ability to be subject to outside environmental conditions occasionally and inside conditions frequently.
