Job Summary
The MTM Pharmacist provides patient-centered MTM services for optimizing medication therapy to improve clinical, economic and humanistic outcomes. The MTM Pharmacist participates in a broad spectrum of MTM services including, but not limited to, Comprehensive Medication Reviews, Targeted Interventions, Adherence Monitoring Interventions, Cost-Effective Interventions, and Transitions in Care to help achieve these outcomes. MTM services may be provided to patients telephonically, by remote video link (telehealth) and/or face-to-face (in-person).
Job Requirements
Minimum Education Requirement:
Doctorate of Pharmacy (PharmD) required.
Minimum Experience Requirement
Two (2) years of acute care or ambulatory pharmacy experience required. Previous experience with providing pharmacy MTM services highly preferred including comprehensive and targeted medication therapy reviews, personal medication lists, and medication action plans. Excellent communication, listening, and interpersonal skills with the ability to develop patient rapport (specifically with elderly population) quickly and effectively. Strong attention to detail and organizational skills. Proficiency using Microsoft Word, Excel, and various EMR systems (such as Meditech Expanse, etc.).
Minimum License Requirement
Current registration and licensure in California as a pharmacist. BLS or ACLS required (ICH/QVH only). Certification by the Board of Pharmaceutical Specialties (BCPS, BCGP, etc.), the National Board of Medication Therapy Management (BCMTMS), or other applicable certifications in areas such as diabetes, geriatrics, opioid management, or medication therapy management preferred.
