LPN- Assistant Health Services Coordinator (ADON)

Elmwood Assisted Living of New Bremen is now hiring for an LPN for the position of Assistant Health Services Coordinator (ADON). This position will work 28 hours a week as Assistant Health Services Coordinator and will require covering shift(s) on the floor, weekly, as an LPN and sharing the on-call rotation with the Health Services Coordinator. Job Summary:

The Assistant Health Services Coordinator is responsible for written plans for each resident that will be developed and monitored. The Assistant Health Services Coordinator works in conjunction with the Health Services Coordinator. Understands and follows all Elmwood policies and procedures with special consideration in understanding the 100 policies. Completes absentee, mandation, call-off forms with accurate documentation for payroll entry as assigned. Completes any other duties as assigned by the Executive Director.

Responsibility For:

Nursing and personal care of the Reminiscence residents and Assisted Living Residents, Reminiscence Programming, and ensuring that good quality care is provided to all residents. Looks after and provides for care and welfare of the residents in a clean, cheerful and healthful environment. Orders supplies and mail order medications that are needed for each resident. Writes and oversees all resident-focused plans. Completes wellness visits. Responsible for off hours and weekend staff coverage in conjunction with the other department leaders as assigned on a rotating schedule per facility rule. Flexes hours to spend time on various shifts. Completes the admission process for new admissions. Completes initial assessments to determine appropriate level of care. Assures completion of the Physician’s Report, admission orders and organizes the medical chart. Completes monthly wellness visits for all residents. Ensure the monthly waiver satisfaction forms are completed and submitted to AOA monthly. Completes monthly ISPs, Shower Schedules, Appetite Logs, and rounds sheets for all residents as part of monthly change-over. Ensures charts are thinned monthly, and all thinned and discharged charts are filed in an organized manner into storage.
