Peer Recovery Coach : Assertive Community Treatment (ACT)

Overview
We are seeking a dedicated and dynamic Assertive Community Treatment (ACT) Team member to provide comprehensive mental health services within a community setting. The ideal candidate will possess a background in behavioral health, crisis intervention, and social work. As the ACT Team member, you will play a pivotal role in ensuring high-quality, client-centered care that promotes recovery, stability, and independence for individuals facing complex mental health and addiction challenges. DUTIES AND RESPONSIBILITIES:

• Supports individuals in acquiring and practicing all activities of daily life.
• Connects individuals to medically necessary and clinically indicated services including DHS, Social Security, food and clothing banks and other community resources.
• Facilitates wellness groups including Wellness Recovery Action Plan (WRAP), Whole Health Action Management (WHAM), Smoking Cessation, among others, and support individuals in those groups in gaining access to community programs and services as they are indicated, especially physical health care and dental services.
• Promotes client decision-making that supports self-determination and participates in the person-entered-planning (PCP) process.
• Assists individuals with the implementation of their goals and objectives identified in the PCP.
• Attend ACT team meetings daily as required by Act Fidelity to review status of all beneficiaries, updates from on-call, clinical and case/care management needs, crisis management, schedule organization, and finalized plans for ACT staff deployment in the community.
• As a team-based service, perform a minimum required weekly contacts as prescribed by Act Fidelity in the beneficiary’s home or other agreed upon community location.
• Attends outings intended to enhance socialization and provides instruction-based assistance including teaching, prompting, modeling, monitoring encouragement and general support to persons served.
• Utilizes a Trauma-Informed and Motivational Interviewing (MI) approaches to deliver services.
• Accounts for scheduled work hours through billing, productivity, or other approved methods of documentation.
• Meet direct service productivity and documentation standards per agency and Act Fidelity expectations.
• Provide transportation assistance to individuals served as needed or when public transportation is not feasible or able to be arranged through the members insurance.
• Participation in on-call rotation as assigned.
• Ensure assigned staff compliance with quality care initiatives including Commission on Accreditation of Rehabilitation Facilities (CARF), SAMHSA Act EBP, Michigan Medicaid Guidelines and Certified Community Behavioral Health Clinic (CCBHC) requirements.
• Ensure that the Elmhurst Home, Inc standards of service are applied to interactions with individuals served, guests, and staff.
• Assume responsibility for professional growth by current reading and attendance at educational programs and conferences.
• Maintain knowledge and compliance with established policies and procedures, corporate compliance program, code of ethics, applicable federal, state, and local laws and regulations, HIPAA standards, and other regulatory programs.
• Perform other related duties as assigned.

Qualifications:

Required

• High school diploma or equivalent.
• Valid and unrestricted Driver’s License with a good driving record.
• State of Michigan certification as a Certified Peer Support Specialist (CPSS) and/or Certified Peer Recovery Coach (CPRC)

or

• Upon eligibility for certification must be willing to become certified by the Michigan Department of Health and Human Services as Recovery Coach within six months of hire.
• Must also possess a Certified Peer Recovery Mentor credential or complete and submit a Development Plan application for this credential to MCBAP within six weeks of hire.
• 1 year paid or volunteer experience working with adults with severe and persistent mental illnesses.
• Prior work or volunteer experience working with adults with severe and persistent mental illnesses.
• Prior experience as a Peer Support Specialist or Certified Peer Recovery Coach.
• Candidates for this position must have experience receiving substance use treatment and/or recovery support services and must have at least two years of sobriety.
• Experience with electronic health record (EHR) systems.
• Must participate in MDHHS-approved, ACT-specific initial training within 6 months of hire, and subsequently participate in at least one MDHHS-approved, ACT-specific training annually thereafter.
• Must be willing to participate in training and supervision to enhance their ability to engage consumers, and to improve their understanding of the fundamental principles of recovery, recovery coaching and self-determination.
• Must demonstrate a non-judgmental approach that reflects cultural competency and sensitivity to the unique needs and diverse experiences of people with substance use disorders and their families.
• Must be able to organize their work, and complete tasks within established timeframes.
• Ability to follow evidence-based programming to achieve required outcome e.g. ACT/IDDT
• Ability to share and teach on the continuum of drug use, such as initiation intoxication, harmful use, abuse, dependence, withdrawal, craving, relapse, and recovery.
• Ability to share and teach about the symptoms of substance use disorders that are similar to those of other medical and/ or psychological disorders and how these disorders interact.
• Working knowledge and understanding of mental illness, medications used in treating psychiatric/SUD disorders, and knowledge and experience with other interventions used to treat psychiatric/SUD disorders.
• Working knowledge of conflict resolution and crisis intervention.
• Proficient with computers and working knowledge of Microsoft office products.
• Ability to work as a member of a multidisciplinary team, and community effectively, professional, and courteously.
• Ability to work closely with persons with mental or physical limitations.
• Ability to contribute to an inclusive environment that recognizes the value and contributions of all persons regardless of race, ethnicity, national origin, gender, religion, age, marital status, sexual orientation, gender identity, or disability.
• Ability to use discretion and judgmental when handling matters of a sensitive or confidential nature.
• Intermediate time management, problem solving, customer service, interpersonal, and conflict resolution skills.
