Elmbrook Home is seeking an experienced and motivated MDS Coordinator to join our leadership team!

Position Summary
The MDS Coordinator is responsible for coordinating and completing the MDS/RAI process while helping ensure accurate resident assessments, individualized care planning, regulatory compliance, and appropriate reimbursement. This position works closely with nursing, therapy, administration, and the interdisciplinary team.
Key Responsibilities
- Coordinate and complete MDS assessments accurately and within required timeframes.
- Manage the RAI process and maintain compliance with CMS requirements.
- Coordinate with the interdisciplinary team to ensure comprehensive and accurate resident assessments and care plans.
- Monitor assessment schedules, ARDs, and completion deadlines.
- Review clinical documentation for accuracy and support of resident acuity and reimbursement.
- Maintain knowledge of PDPM, Medicare requirements, and changes affecting MDS and reimbursement.
- Participate in Medicare meetings, IDT meetings, Triple Check, QAPI, and other clinical/operational meetings as needed.
- Work collaboratively with nursing, therapy, medical records, business office, and facility leadership.
- Assist with survey readiness and ensure MDS-related documentation meets regulatory requirements.
- Identify opportunities to improve documentation, quality measures, and resident outcomes.
Qualifications
Current Oklahoma nursing license required and previous MDS experience in a skilled nursing facility . Candidates should have a working knowledge of the RAI process, PDPM, Medicare, care planning, and long-term care regulations. RAC-CT certification is a plus but not required.
