Elliott Concierge Physical Therapy is seeking an experienced Physical Therapist to join our growing concierge practice in Naples.

This opportunity is designed for a clinician who is confident working independently, communicates exceptionally well, and wants to provide high-level care without the volume and constraints of a traditional outpatient clinic.

We provide private, one-on-one physical therapy directly in patients’ homes, condominiums, clubs, and senior-living communities throughout Naples.

Compensation & Caseload

Your first 8 weeks are protected by a $2,000/week guaranteed minimum while your caseload is being built.

• $100 per completed standard patient visit
• $200 per approved evening, weekend, or after-hours visit
• $200 weekly bonus at 20 visits
• $350 weekly bonus at 25 visits
• $500 weekly bonus at 30 visits
• Target established caseload: approximately 20–30 visits per week

Examples using standard visits:

20 visits = $2,200/week - effectively $110/visit

25 visits = $2,850/week - effectively $114/visit

30 visits = $3,500/week - effectively $117/visit

Premium evening and weekend appointments can increase compensation further.

If your visit-based compensation and applicable bonuses exceed the guaranteed minimum during the first 8 weeks, you earn the higher amount.

Why This Position Is Different

ECPT operates differently from both a traditional outpatient clinic and Medicare home health.

Our therapists provide one-on-one, primarily 60-minute visits directly where patients live.

That means:

• No double-booking
• No crowded clinic schedule
• No traditional insurance authorization or visit-limit pressure
• No OASIS
• No Medicare home-health model
• Significant clinical autonomy
• Time for hands-on treatment, exercise progression, education, and meaningful patient communication

The goal is to allow an experienced therapist to provide excellent care while building a substantial and financially rewarding caseload.

Our Patients

Because our specialty is the concierge delivery model rather than one specific diagnosis, our caseload is intentionally broad.

Patients may include:

• Post-operative orthopedic patients
• Adults with musculoskeletal pain and injuries
• Balance and fall-risk patients
• Vestibular patients
• Neurological patients
• Older adults working to maintain mobility and independence
• Golf and tennis athletes
• Active adults returning to exercise or sport

Many of our patients are highly engaged and accustomed to a high level of professional service. Strong communication, professionalism, clinical confidence, and follow-through are essential.

The Clinician We Are Looking For

We are looking for someone who wants to build a meaningful caseload and take ownership of their patients, rather than simply pick up occasional PRN visits.

The strongest candidate will:

• Be clinically confident and comfortable practicing independently
• Communicate exceptionally well with patients, families, and our team
• Be reliable, responsive, and organized
• Have strong clinical reasoning skills
• Be comfortable treating a broad adult population
• Have hands-on/manual therapy skills
• Be willing to research unfamiliar but appropriate diagnoses when needed
• Take ownership of patient care, communication, and continuity
• Be comfortable traveling throughout the Naples area
• Want to build toward approximately 20–30 visits per week

Strong generalists are welcome, as are clinicians with advanced expertise in areas such as orthopedics, manual therapy, vestibular rehabilitation, neurological rehabilitation, balance, pelvic health, sports rehabilitation, or golf.

Specialty expertise is a plus, provided the clinician is also comfortable supporting the broader ECPT caseload when appropriate.

Clinical Setting & Schedule

Patients are treated in settings such as private homes, condominiums, country clubs, independent-living communities, assisted-living communities, and memory-care settings when clinically appropriate.

This is mobile outpatient physical therapy, not traditional Medicare home health.

Previous mobile or home-health experience is not required.

Because this is a mobile practice, therapists should be comfortable traveling between patient locations throughout the Naples area. Individual drives approaching approximately 30 minutes may occasionally occur.

Some flexibility is important. Occasional early-morning, evening, or weekend appointments may be available or requested. Approved premium appointments are compensated at $200 per visit.

A caseload of approximately 30 visits per week represents a full clinical schedule once treatment time, travel, documentation, and patient communication are considered.

Qualifications

• Active, unrestricted Florida Physical Therapist license, or ability to obtain one prior to starting
• Doctor of Physical Therapy preferred
• 2 years of clinical experience preferred
• Strong clinical reasoning and communication skills
• Orthopedic and manual-therapy experience strongly preferred
• Comfortable treating a broad adult patient population
• Reliable transportation
• Ability to travel between patient locations throughout Naples
• Professional, dependable, and comfortable operating independently

Exceptional clinicians with less than two years of experience may also be considered.
