Job Summary:
As a Social Worker, you will play a vital role in providing psycho-social support to our residents and their families. Responsibilities:
- Assess the emotional, social, and psychological needs of residents and develop individualized plans to address their concerns
- Provide psycho-social support to residents and their families, focusing on emotional, social, and psychological well-being
- Collaborate with other healthcare professionals to develop and implement personalized care plans
- Identify and address any emotional, social, or psychological issues that may impact a resident's recovery or quality of life
- Maintain accurate and detailed records of resident interactions and care plans
- Participate in interdisciplinary team meetings to share knowledge and expertise, and to ensure seamless communication among team members
Requirements:
- Bachelor's degree in Social Work
- Minimum 1 year of experience in social work or a related field
- Strong communication and interpersonal skills, with the ability to build rapport with residents, families, and healthcare professionals
- Ability to work effectively in a fast-paced, dynamic environment
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities
