Job Description
Eliza at Chagrin Falls, a New Eliza Jennings Community in Chagrin Falls is searching for full-time Community Relations Coordinator to provide care in the skilled nursing facility.
Hours for this position are 8:30p-5p M-F. Weekend availability for events as needed.
Position Summary:
The Community Relations Coordinator assists in ensuring a smooth, person-centered experience for residents and families throughout the continuum of care on campus. This role supports the interdisciplinary team by assisting in the coordination of admissions, transfers, and discharges, and serves as a liaison for residents and families. The position also contributes to census development and community outreach under the guidance of the Director of Sales & Marketing.
Essential Duties & Responsilbities:
Sales & Marketing
- Assist with marketing initiatives, tours, and inquiry follow-up as directed by the Director of Sales & Marketing.
- Develop and maintain a referral network by engaging with community partners and healthcare professionals.
- Maintain accurate records, track leads, and conduct follow-ups per defined protocols.
- Participate in outreach activities, health fairs, and community events to promote Eliza Jennings services.
- Deliver campus and community presentations and support marketing campaigns designed to attract prospective residents.
- Keep current with competitors' offerings and highlight unique features of Eliza Jennings’ continuum of care.
Resident Experience & Care Coordination
- Assists residents and families during move-in, care transitions, and discharge.
- Assists in the coordinate the admission, transfer, and discharge process across care levels, ensuring timely and complete documentation.
- Continued support resolution of resident or family concerns and serve as a liaison to admissions, clinical and support departments.
- Collaborate with staff across departments to enhance the overall resident experience and ensure continuity of care.
Administrative & Communication Responsibilities
- Maintain positive relationships with residents, families, referral sources, and staff.
- Assist in preparing internal and external communications, correspondence, reports, and presentations.
- Ensure a safe, inclusive environment that respects each individual’s autonomy and choices.
- Adhere to HIPAA and all relevant policies and procedures.
- Complete required in-service education and participate in external training opportunities.
- knowledge of the Network’s services, facilities and expertise, including distinguishing features.
- Maintain current knowledge of competitors’ services, facilities, and expertise, including distinguishing features.
- Complete in-service training: participate in external sales training as opportunities present themselves.
- Provide the people we serve with clear opportunities to direct their own lives and decide how they will spend their day.
- Ensure a safe environment for residents and utilize tools and equipment in a safe manner.
- Report on all hazardous conditions including incidents, injuries, and equipment to the appropriate Supervisor immediately.
- Adhere to mission statement and all company policies including, but not limited to job description, HIPAA, Residents’ Rights, annual TB test, and education assignment completion.
- Participate on committees as assigned.
- Adapt to changes in responsibilities in a positive manner, be receptive to new ways of completing tasks, and flexible in carrying out assignments.
Qualifications:

- Bachelor’s degree with focus in health care and/or marketing.
- Demonstrated sales proficiency.
- Understanding business, budgeting, and database management.
- In-depth knowledge of local, state, and federal regulations.
- In-depth communication skills: demonstrated ability to present material orally and in writing in an articulate and concise manner; demonstrated ability to listen and process others’ points of view.
- In-depth emotional intelligence: behavioral maturity, demonstrated trust and integrity, high ethical standards, demonstrated self-awareness and accurate self-assessment; demonstrated ability to prevent, reduce, and resolve conflict; sensitivity and empathy, openness, reliability and consistency.
- Computer proficiency with understanding of business, professional and industry related applications.
- Demonstrated ability to negotiate and close business, recognize alternatives, evaluate relevant information and make timely decisions.
- Demonstrated ability to manage multiple, complex priorities with demanding timeframes, strong organizational skills.
- Preferred but not required: Licensed Independent Social Worker (LISW) or Ohio Licensed Nursing Home Administrator (LNHA).
