Job Summary
The Social Service Coordinator plays a vital role in supporting individuals with developmental disabilities by coordinating services, developing programs, and fostering positive relationships within the community. This position requires a compassionate and organized professional dedicated to enhancing the quality of life for clients through effective management and social work practices. The coordinator will oversee program development, supervise staff, and collaborate with families, service providers, and community organizations to ensure comprehensive support tailored to each individual's needs.
Responsibilities
- Develop, implement, and evaluate programs designed to meet the needs of Seniors 62 years and older in indedependent housing.
- Manage daily operations of social services, ensuring compliance with organizational policies and regulatory standards i.e. ADL's
- Supervise community volunteers on -site.
- Build and maintain strong relationships with clients, families, service providers, and community partners to facilitate healthy engagements
- Coordinate care assessments to identify client needs and develop individualized service plans to identify the ability to independently live.
- Oversee budgeting processes related to program activities and resource allocation.
- Maintain accurate documentation of client interactions, program activities, and administrative records.
- Complete weekly Reports
- Advocate for clients’ rights and connect them with appropriate resources such as Social Security, Financial Institutions, Health Care Providers, Medical Providers, Transportation Providers. Cellular and Digital Services.
- Monitor program outcomes and implement improvements based on feedback and data analysis.
Skills
- Proven management and program development experience within social services or related fields.
- Demonstrated ability to work effectively with Seniors
- Strong relationship management skills to engage clients, families, community partners, and team members.
- Excellent administrative skills including record keeping, reporting, and budgeting.
- Supervising experience with the ability to lead a diverse team in a dynamic environment.
- Background in social work or behavioral health is highly desirable.
- Exceptional organizational skills with attention to detail in program management tasks.
- Compassionate communication skills tailored to working with vulnerable populations.
