- Delivers high-quality therapy services in accordance with the relevant Standards of Practice
- Conducts patient evaluations and formulates individualized treatment plans, encompassing both short-term and long-term objectives
- Monitors and communicates the patient's progress, responses to treatments, and any changes in their condition to the responsible physician; adjusts the treatment plan as necessary
- Provides guidance to the patient, caregiver, and other healthcare team members on strategies to enhance and correct disabilities
- Maintains regular communication with physicians and the broader healthcare team
- Facilitates patient discharge planning by establishing realistic and achievable long-term goals that can be adjusted as the patient's condition evolves
- Prepares the patient and their caregiver for post-discharge care
- Reports any significant concerns related to patients, families, or physicians to the Clinical Director
- Must hold current state licensure, CPR certification, driver's license, and automobile liability insurance
- Thorough knowledge of professional theory, practice, techniques, principles, and equipment
- Effective oral and written communication skills. Ability to document patient care effectively
- Ability to establish and maintain a working relationship with patients, other health care professionals, physicians, and the public
- New Grads considered, 1 year of experience preferred
