CHOOSE THE SCHEDULE THAT WORKS FOR YOU!
FULL-TIME RN CASE MANAGER
- $80,000 annual base salary performance bonus
- Monday–Friday, 8:00 AM–5:00 PM
- Rotating on-call responsibility approximately one weekend per month
- Structured orientation, clinical support, and opportunities for professional growth
PART-TIME WEEKEND RN
- $40 per hour mileage
- Every other weekend
- Home health experience required
- Some weekday training required

As an RN Case Manager, you’ll oversee patient care, coordinate services, support clinical excellence, and help patients achieve the best possible outcomes.
WHAT YOU’LL DO
- Manage patient care from admission through discharge
- Complete comprehensive assessments, OASIS admissions, recertifications, resumptions of care, and discharge assessments
- Develop, implement, and oversee individualized Plans of Care
- Coordinate care with physicians, therapists, social workers, home health aides, patients, and families
- Provide clinical guidance, education, and support to nursing staff
- Perform home visits and monitor patient progress
- Apply your wound care knowledge, including wound vac management, when indicated
WOUND CARE EXPERIENCE IS A PLUS — WE’LL TRAIN THE RIGHT RN!
Have wound care or wound vac experience? Excellent! Want to strengthen those skills? We’re willing to provide training to the right candidate.

WHO WE’RE LOOKING FOR
- Active Registered Nurse (RN) license in Nebraska
- Strong clinical assessment, communication, and organizational skills
- Confidence working independently while collaborating with an interdisciplinary team
- A commitment to quality care, patient advocacy, and professional excellence
- Home health experience required for the part-time weekend position
