We are seeking a dedicated and energetic Nurse Practitioner or Physician Assistant to join our healthcare team. Responsibilities
- Conduct thorough patient assessments, including physical examinations, vital signs, and medical history reviews to determine diagnoses and treatment plans.
- Manage acute and chronic conditions across multiple specialties including emergency medicine, geriatrics, pediatrics, and behavioral health.
- Ensure proper documentation in electronic health records (EHR) systems
- Collaborate with multidisciplinary teams on discharge planning, case management, utilization review, and telehealth services to optimize patient care pathways.
- Assist in diagnostic evaluations utilizing laboratory experience, medical imaging, sonography, and specimen collection/processing techniques.
- Educate patients on health coaching topics such as nutrition, disease management (including infectious disease care), medication adherence, and post-acute care strategies.
- Maintain compliance with HIPAA regulations and ensure accurate medical documentation following ICD coding (ICD-9/10), CPT coding standards, and NCQA standards.
Skills
- Proficiency in EMR/EHR systems including Epic, Cerner, Athenahealth or similar platforms.
- Extensive clinical experience in diverse settings such as hospital medicine, emergency room (ER), urgent care, outpatient clinics, nursing homes or assisted living facilities.
- Strong knowledge of physiology and anatomy for effective patient assessment and diagnostic evaluation.
- Skilled in performing advanced procedures like spinal taps, cardiac catheterization assistance, ventilator management, gastrostomy tube feeding or NG tube placement.
- Experience working with special populations including children (pediatrics), seniors (geriatrics), individuals with developmental disabilities or dementia care needs.
- Familiarity with acute pain management techniques and infection control protocols across various healthcare environments.
- Knowledge of Workers' Compensation law related to occupational health cases along with hospice & palliative medicine practices.
- Competence in medical documentation including ICD coding (ICD-9/10), CPT coding for procedures and treatments.
