About the Patient:
We are seeking a compassionate and reliable nurse to join our home care team for a vibrant and active 10-year-old girl with complex medical needs. The patient is post multi-visceral transplant and requires skilled nursing care to support her day-to-day treatment, medication administration, and safety.
Key Responsibilities:
- Supervise and ensure the safety of the patient at all times
- Administer scheduled medications via G-tube and central line with strict adherence to timing and technique
- Perform straight catheterization every 3 hours (3 PM, 6 PM, 9 PM)
- Monitor and document intake/output (ostomy, G-tube, urine) in Axxess
- Conduct ostomy and G-tube care per care plan
- Engage the patient in age-appropriate play and developmentally supportive activities
- Maintain infection control and cleanliness of the environment
- Complete evening ADLs (assisting with hygiene and bedtime prep)
- Communicate clearly with the patient’s family and clinical team
Ideal Candidate Will Have:
- MA RN or LPN license (required)
- Pediatric and/or home health experience (preferred)
- Proficiency in G-tube, ostomy, and CVL care
- Ability to follow detailed care plans and document care accurately
- Strong communication skills and a patient-centered mindset
