Duties
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts and transfer devices
- Monitor and document patient observations, vital signs, and changes in condition accurately within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
- Provide medication reminders and assist with medication administration following medical terminology guidelines and HIPAA privacy standards
- Support patients with specialized care needs such as Alzheimer’s, dementia, memory care, and developmental disabilities by implementing tailored care strategies
- Perform catheter care, wound care, and other medical procedures under the supervision of nursing staff while maintaining safety protocols
- Engage in light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe environment aligned with infection control practices
- Document all patient interactions thoroughly in medical records and ensure compliance with healthcare regulations
Skills
- Strong ability to observe and report on patient conditions accurately and promptly
- Experience working in assisted living facilities, nursing homes, or home health environments with seniors or individuals with disabilities
- Familiarity with EMR/EHR systems for medical documentation and record keeping
- Knowledge of medical terminology, CPR certification, and basic first aid procedures
- Skilled in assisting with ADLs such as feeding, grooming, mobility assistance, and hygiene routines
- Experience providing hospice care or working with patients facing end-of-life challenges with sensitivity and compassion
- Ability to operate medical equipment like Hoyer lifts, blood pressure cuffs, thermometers, and other vital signs monitoring tools
