What are the responsibilities and job description for the RN or LPN Home Health Field Nurse position at Elite Home Healthcare?
Imagine starting your day not in a crowded hospital hallway, but on the road—coffee in hand—heading to care for patients where they feel most comfortable: home.
We’re looking for a compassionate, skilled Home Health Nurse who values connection just as much as clinical excellence. In this role, you won’t just treat conditions—you’ll build relationships, restore independence, and make a real difference in people’s daily lives.
As part of our team, you’ll provide one-on-one care to patients with a variety of needs, from post-surgical recovery to chronic disease management. You’ll assess, plan, and deliver skilled nursing care while working closely with physicians, patients, and families to ensure the best outcomes. Every home you enter brings a new story, a new challenge, and a chance to leave someone better than you found them.
This role is ideal for a nurse who thrives in an autonomous setting but still values strong team support.
We’re Looking For Someone Who Is
Licensed (RN or LPN) with current state credentials
Experienced in or passionate about home health or community-based care
Compassionate, dependable, and confident working independently
In return, we offer a supportive environment, flexible scheduling, competitive pay, and the opportunity to practice nursing the way it was meant to be—personal, meaningful, and impactful.
If you’re ready to step outside the hospital walls and into a role where your care truly meets patients where they are, we’d love to meet you.
Full Time and Part Time hours available.
