We are seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes under the direction of a Registered Nurse (RN).

Position Summary
The LPN provides skilled nursing services in accordance with physician orders, established care plans, RN direction, agency policies, and applicable state and federal regulations.
Responsibilities
- Provide skilled nursing care to patients in the home setting
- Administer medications and treatments within LPN scope of practice
- Perform wound care, dressing changes, catheter care, and other skilled nursing tasks
- Monitor patient condition and report changes to the supervising RN
- Reinforce patient and caregiver education
- Follow established plans of care and physician orders
- Complete accurate and timely documentation in the EMR
- Communicate with RNs, physicians, and other members of the care team
- Participate in required training, meetings, and in-services
- Provide care in accordance with Medicare requirements, state regulations, and agency policies
Qualifications
- Graduate of an accredited Practical Nursing program
- Current Florida LPN license in good standing
- Minimum 1 year of nursing experience preferred
- Home health, hospice, or acute care experience preferred
- Current CPR certification
- Valid driver's license, reliable transportation, and auto insurance
- Ability to meet all required background and compliance requirements
