We are seeking a skilled and compassionate Physical Therapist (PT) to join our Home Health team. This role involves conducting visits to assess and treat clients, helping them regain function and improve their quality of life.
The ideal candidate will have experience in home health care and be comfortable managing caseloads independently while maintaining high clinical standards.
Key Responsibilities:
Perform initial assessments, reassessments, and discharge evaluations for home health clients.
Develop, implement, and update the Medical Plan of Care in coordination with the client’s physician.
Ensure timely and accurate documentation of patient evaluations, goals, and progress notes.
Communicate directly with physicians and other healthcare providers regarding patient status, progress, and any changes in the treatment plan.
Supervise and provide clinical oversight to Physical Therapy Assistants (PTAs), ensuring compliance with care plans and quality standards.
Monitor patient outcomes and adjust treatment plans as needed.
Maintain compliance with all federal, state, and agency regulations for home health services.
