Responsibilities
- Coordinate patient care plans by collaborating with multidisciplinary teams including nursing, physicians, social workers, and specialists.
- Conduct comprehensive patient assessments, including vital signs and physiological evaluations, to determine care needs.
- Manage discharge planning and post-acute care arrangements to ensure smooth transitions from hospital to home or other facilities.
- Oversee case management activities such as utilization management, diagnostic evaluation, and medical record review to optimize resource use.
- Facilitate intake processes for new patients and maintain accurate documentation in medical records while ensuring compliance with HIPAA regulations.
- Assist with patient education on medical terminology, treatment plans, and health management strategies to empower informed decision-making.
Experience
- Proven experience in critical care environments such as ICU, Level I or Level II trauma centers, or crisis care or emergency medicine settings.
- Strong background in crisis care management or outpatient clinics with familiarity in acute care and post-acute settings.
- Experience working with diverse populations including geriatrics, pediatrics (PICU), behavioral health patients, and infectious disease cases.
- Knowledge of physiology principles and medical terminology essential for accurate patient assessment and documentation.
- Prior involvement in discharge planning, inpatient/outpatient case management or utilization management programs preferred.
- Demonstrated ability to navigate hospital records systems while maintaining strict adherence to HIPAA standards.
- Excellent communication skills combined with a proactive approach to patient service and team collaboration.
