Job Summary

Elite Concierge Alliance is looking for an amazing nurse in the Santa Barbara area. This is a pay-per-visit contractor position, no benefits provided. The concept behind this company is to provide "health and wellness coaching," as we do not officially provide medical advice. That being said, we aim to prevent hospitalizations by identifying issues and addressing them with teaching, televisits with a PCP, lab draws, connecting to other services, etc. Each patient is different, obviously, and we are looking for a trustworthy, experienced nurse who is comfortable working independently and enjoys problem solving, can accurately assess root causes of problems, and can navigate patients and families that are usually wealthy. This is a cash payment business. There are many benefits to this, primarily that we only work for the patient, not insurance, not physicians, not healthcare organizations. Another benefit is charting. Our charting is fairly freehand, vitals, meds, relevant diagnoses, and a narrative. We want you focused on actually helping the patient. This is a position that will have only a few visits per week to start, but is expected to be full time with growth.

We are seeking a compassionate and highly skilled Home Concierge Nurse to provide personalized healthcare support directly in patients’ homes. This dynamic role combines clinical expertise with exceptional patient service, ensuring individuals receive comprehensive, coordinated care in a comfortable and familiar environment. As a Home Concierge Nurse, you will serve as a trusted healthcare partner, guiding patients through their health journeys, managing complex medical needs, and collaborating seamlessly with healthcare professionals to optimize outcomes. Duties

• Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine individual needs.
• Manage acute and chronic conditions by developing personalized care plans that incorporate hospital medicine, post-acute care, and home health strategies.
• Coordinate discharge planning and facilitate smooth transitions from hospital or clinic settings to home-based care environments.
• Utilize Electronic Health Records (EHR) systems to document patient interactions accurately while maintaining strict adherence to HIPAA regulations for patient privacy and data security.
• Collaborate with multidisciplinary healthcare teams—including physicians, therapists, social workers, and specialists—to ensure comprehensive patient care and effective communication.
• Provide education on disease management, medication adherence, infection control (including infectious disease care), and health regulation policies to empower patients in managing their health.
• Perform diagnostic evaluations and manage case documentation for complex cases such as behavioral health issues, geriatrics, pediatrics, or hospice care.
• Assist with intake procedures for new patients and manage ongoing clinical case management to optimize health outcomes across inpatient, outpatient, and home settings.
• Support managed care initiatives by participating in utilization management activities and ensuring appropriate resource allocation for each patient’s needs.

Qualifications

• Valid nursing license with current registration; experience in multiple care settings preferred.
• A diverse backround in nursing is desired, but home health experience is ideal.
• Proven background in hospital medicine, infectious disease care, behavioral health, geriatrics, or pediatric intensive care units (PICU).
• Strong knowledge of physiology principles, medical terminology, HIPAA compliance, and electronic health records (EHR/EHR management).
• Experience working within hospital environments or home health agencies with inpatient or outpatient focus.
• Skilled in patient assessment techniques including vital signs monitoring and diagnostic evaluation.
• Ability to manage complex clinical cases through effective case management strategies and discharge planning processes.
• Excellent client communication skills with the ability to collaborate effectively with healthcare professionals across various disciplines.
• Familiarity with health regulation policies and managed care practices; knowledge of medical-surgical procedures is beneficial.
