Role Description
This is a part-time/PRN on-site role for a Visit Nurse at Elite Care Management. As a Home Health Visit Nurse, you will be responsible for providing compassionate and personalized care to patients in their homes. Your day-to-day tasks will include conducting assessments, administering medications, coordinating with other healthcare professionals, and educating patients and their families on care plans.

Responsibilities:
- Provide comprehensive patient care during shifts, including catheterization, administering medication, and dressing changes.
- Conduct thorough patient assessments and monitor vital signs to ensure safety and well-being.
- Utilize EMR systems for accurate documentation and maintain compliance with health regulations.
- Collaborate with healthcare teams to develop and implement individualized care plans.
- Administer basic life support and respond to emergencies as needed.
- Ensure a clean and safe environment for patients.
- Ability to sit, stand, and kneel as needed to perform physical assessment
- Ability to work independently and part of a team
- Indiana Registered Nurse (RN) license
- Experience preferred in home health care or a similar setting & Oasis Documentation
- Strong communication and interpersonal skills
- Knowledge of medical equipment and procedures
- Compassionate and empathetic demeanor
- Attention to detail and organizational skills
- Valid driver's license
- CPR Certification
