Position Overview
As an RN Floater / Case Manager, you will be cross-trained across multiple patient cases, providing both hands-on skilled nursing care and case management oversight. This includes supporting coverage needs while ensuring high-quality, compliant patient care.
Key Responsibilities
- Perform supervisory visits and routine skilled nursing visits
- Provide shift coverage as needed across assigned cases
- Manage and coordinate patient care plans in collaboration with physicians and interdisciplinary team
- Monitor patient progress and update care plans accordingly
- Ensure compliance with all clinical, documentation, and regulatory standards
- Serve as a clinical resource for field staff and caregivers
