Duties

• Deliver patient observation and monitor vital signs to ensure safety and well-being.
• Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support.
• Administer medications and perform catheter care following medical protocols and HIPAA regulations.
• Operate Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient information accurately.
• Support patients with dementia or Alzheimer's by implementing specialized memory care techniques and engaging activities.
• Use equipment such as Hoyer lifts and manage patient transfers safely and comfortably.
• Prepare nutritious meals, assist with cooking, laundry, cleaning, and maintaining a clean living environment.
• Document medical records meticulously, including medication administration and patient progress notes.
• Provide compassionate hospice care when needed, respecting patients’ dignity and comfort preferences.
• Collaborate with interdisciplinary teams to develop personalized care plans tailored to each individual’s needs.

Requirements

• Valid HHA or CNA certification from an accredited program; prior experience in nursing homes or assisted living is highly preferred.
• Demonstrated experience working with individuals with developmental disabilities or in home & community care settings.
• Knowledge of medical terminology, medication administration procedures, and medical documentation standards.
• Ability to perform patient observation accurately and respond promptly to changing conditions.
• CPR certification is required; additional certifications such as hospice care or dementia training are a plus.
• Experience operating EHR/EMR systems and familiarity with HIPAA compliance for patient privacy protection.
• Strong caregiving skills with a compassionate approach to senior care, memory care, and working with individuals with disabilities.
• Physical ability to lift patients using equipment like Hoyer lifts; capable of standing, walking, bending, and assisting with mobility tasks. Pay: $15.00 - $18.00 per hour
