Position Summary:
The Licensed Practical Nurse is an important team member. The skills performed by the Licensed Practical Nurse serves the physical, mental, and spiritual needs of the resident. The combining of these skills, with an understanding of the wide scope of psycho-social intervention, provides for total individual resident care.
Qualifications:
- Current LPN Licensure in State.
- CPR, and Specialty training for Dementia and Mental Health.
- Washington Food Handle Permit.
- 2 years experience and knowledge in Assisted Living/Memory Support preferred.
- Experience in supervising a team of Nursing Assistants preferred.
Essential Duties and Responsibilities:
- Creates and maintains an environment that translates the mission of eliseo into actions that are conducive to the maximum well-being of the residents served.
- Assures that department services are of appropriate quality and are consistent with eliseo philosophical principles.
- Facilitates coordination among all departments & divisions of eliseo in delivery of services.
- Ensures consistent action is taken by all employees, volunteers, and visitors to comply with HIPAA security/privacy regulations regarding confidential patient records and information.
- Assists with assessments, development, and implementation of resident service plans.
- Performs specific treatments and care, as ordered by the physician or authorizing practitioner. Coordinates medications with pharmacies.
- Administers prescribed medications and supervises administration of medications by caregivers.
- Performs and/or assists the residents with all personal care activities.
- Participates in resident/family/caregiver teaching.
- Maintains documentation of all nursing activity, performance, and observations, according to the requirements of individual care needs.
- Communicates with providers and families regarding resident condition changes.
- Protects the privacy of residents and employees.
- Represents the facility and nursing services in professional and community activities as assigned.
- Perform conflict resolution and administers disciplinary action among employees.
- Responsible for coordinating resident admissions and discharges
- Maintains a positive working relationship between nursing staff and other departments.
SUPERVISORY RESPONSIBILITIES:
Responsible for the overall direction and coordination of Nursing Assistant Staff. The supervisor will carry out responsibilities in accordance with eliseo policies and applicable laws. Responsibilities include orientation, training, planning, assigning, and directing work; appraising performance; addressing complaints and resolving problems.

Upon acceptance of an offer, candidates must be able to pass a drug screen, reference checks, and background check prior to starting work with eliseo. Upon first day of hire, all staff are required to provide documentation of influenza vaccination (Seasonally for Influenza). Additionally, all staff will undergo tuberculosis testing upon their first day and annually.
