Eliot is seeking an energetic and empathic Health Home Care Manager to support our behavioral health clients in medical and mental health needs. Eliot's Health Home program is an innovative care management approach for adults with complex medical, behavioral health, and social needs. The ideal candidate for this position will have a strong commitment to mental health recovery, knowledge of substance abuse disorders and commitment to coordination and integration of all aspects of health care. Additionally, the candidate should possess strong time-management, organizational and communication skills. |
