Responsibilities of MDS Coordinator:
- Gather information on our skilled nursing facility's current and future patients for Minimum Data Set assessments, including physical and mental status, in coordination with the facility's Interdisciplinary Team to ensure comprehensive and accurate assessments are submitted in accordinance with state, federal and RAI standards.
- Participate in the admission process of prospective residents to determine skilled care needs, including potential PDPM groupings, diagnosis coding, nursing and rehabilitative needs and forecasts, etc.
- Maintain current working knowledge of Medicare criteria, serving as a resource for nursing staff and communicating changes in regulations
- Monitor Case Mix Index (CMI) scores, looking for potential risks and/or changes that may affect Medicaid reimbursement
- Create, maintain and monitor MDS assessment schedules for a wide range of payors (Medicare, Medicaid, Managed Care, Private, Hospice, etc.) to ensure timely submission
- Assess charts and communicate with healthcare teams to create applicable health care plans for their current and incoming residents.
Qualifications for MDS Coordinator:
- Must have current, unencumbered Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Massachusetts
- 1 year nursing experience
- Experience in a clinical and healthcare setting
- Deep medical terminology knowledge
- Excellent leadership skills
- Demonstrable knowledge of state and federal regulations
- Superb decision-making skills
- Proficiency in MDS 3.0
- MDS, Medicare, and Medicaid experience
