About the Position
An established hospital in the Las Vegas area is seeking an experienced Nurse Case Manager to support coordinated, patient-centered care across the healthcare continuum. This role works closely with patients, families, physicians, and interdisciplinary teams to help ensure safe, effective, and timely care from admission through discharge.
The Nurse Case Manager serves as a clinical resource and advocate, helping assess patient needs, coordinate services, and guide care planning in a fast-paced hospital environment.
What You’ll Do
- Coordinate patient care using a multidisciplinary approach that supports quality outcomes and safe transitions of care
- Assess patient needs and develop care and discharge plans based on clinical, psychosocial, and payer requirements
- Work closely with physicians, nursing staff, and support services to align care plans and utilization goals
- Provide education and support to patients and families regarding treatment plans, resources, and next steps
- Review clinical documentation and assist with utilization review and payer communication as needed
- Serve as a resource to staff on case management processes, patient flow, and regulatory requirements
- Advocate for patients while balancing quality, safety, and appropriate resource use
What We’re Looking For
- Graduate of an accredited school of nursing
- Active Nevada RN license
- BLS certification through the American Heart Association
- At least three years of nursing experience in an acute care hospital
- One year of experience in case management, discharge planning, or utilization review
- A master’s degree in nursing with a focus on case management may substitute for experience at the hospital’s discretion
Experience That’s Helpful
- Three or more years of case management experience in an acute care setting
- Recent hands-on experience in an acute care environment
- Pediatric case management experience is preferred
- Emergency Department experience is a plus
- Utilization review experience is a plus
