What are the responsibilities and job description for the RN Home Health Coordinator - OFFICE ROLE position at Elios Healthcare?
Home Health Nurse Coordinator - OFFICE ROLE
Multi-site community health center | Northeastern Massachusetts | Full-time | Office-based
**Perfect for an experienced Home Health RN that wants an office position vs being in the field
About the Role
You will help coordinate the home health program for a community health center: every referral, from the provider's order to the first visit and through recertification. Patients count on you to get the right services started on time, and providers count on you to catch the problems before they reach the patient.
This is an office-based nursing role with no patient assignment. You put years of home health experience to work from a desk, coordinating with providers, care teams, patients, and home health agencies by phone, EHR, and in team meetings. Your clinical judgment stays at the center of the job, and the driving from home to home does not.
Why Nurses Take This Role
- An office job, as a nurse. Office-based work with no patient load and no daily visit route.
- Your experience is the job. Your home health background is the core qualification here, and you use all of it.
- Ownership. You run the program end to end, from referral review to follow-up with agencies.
- A voice in care decisions. You sit in care team and population health meetings and help shape how the health center supports patients at home.
- Mission you can see. Community health center work means your coordination keeps the health center's own patients safe and supported at home.
What You Will Do
Referral management
- Coordinate and manage all home health referrals.
- Review referrals to confirm services are medically necessary and appropriate.
- Track active referrals, services, and required documentation and orders.
- Monitor certification and recertification so services remain appropriate.
Care coordination
- Work with providers, care teams, patients, and home health agencies so services start on time and follow the patient's care plan.
- Follow up with home health agencies on delays, missed visits, or concerns.
- Help coordinate care during patient transitions and hospital discharges.
- Educate patients and caregivers about home health services and their care plans.
Quality and population health
- Identify care gaps, safety concerns, and opportunities to improve services.
- Participate in care team and population health meetings.
What You Bring
- Bachelor of Science in Nursing (BSN).
- Current Massachusetts RN license.
- 5 years of clinical nursing experience, including at least 3 years in home health nursing.
- Strong clinical assessment and care coordination skills.
- Comfort working with home health agencies, providers, patients, and care teams, with the communication skills to build strong working relationships.
- Strong organization across multiple referrals and priorities.
- Experience with EHR systems and Microsoft Office.
- Valid driver's license and reliable transportation.
Nice to Have
- Utilization management experience.
- Experience in a community health center or primary care setting.
- Familiarity with local home health agencies and referral processes.
Is This You?
If you know home health from the inside, including which agencies follow through, what a clean order looks like, and where referrals stall, this role puts that knowledge to work for an entire patient population. You get an office, a program to own, and a care team that relies on your judgment.
