A well-regarded Houston-area health system is hiring a Manager of Case Management to lead day-to-day operations across the case management and discharge planning functions. This is a leadership role with direct impact on patient outcomes, length of stay, and care transition quality.
What you'll bring:
- Active RN licensure in Texas or LMSW (LCSW preferred)
- Case Manager Certification (CCM or equivalent) required
- 5 years in utilization management, case management, discharge planning, or a related cost/quality program
- 3 years of hospital-based nursing or social work experience
- 3 years of demonstrated leadership experience
Compensation: $108,000 - $130,000 annually, commensurate with experience
