Duties and Responsibilities
Because the success of Elim Care depends on the dedication of our employees, we are highly selective in choosing new members of our team. The Resident Care Coordinator (RCC) plays a critical role in enhancing the quality of life for residents at the community. The RCC is responsible for overseeing all caregiving staff, ensuring the delivery of high-quality care in compliance with California state laws and regulations while fostering a compassionate, supportive environment for both residents and their families. This position may involve working evenings, weekends, and holidays. The RCC must be available to respond to emergencies and provide necessary support to residents and staff. Duties and responsibilities include:
- Recruit, train, supervise, and evaluate caregiving staff to ensure compliance with facility policies and California state regulations
- Support leadership team in developing training programs specific to dementia and Alzheimer's care interventions and ensure ongoing staff education
- Conduct regular performance reviews and provide feedback to improve caregiver staff performance and service delivery
- Monitor staffing hours and overtime daily to detect problems early
- Avoid utilizing temporary staffing without approval of Executive Director in advance
- Promote a culture where each resident is of the greatest importance
- Coordinate and oversee individualized care plans for each resident, ensuring that care is tailored to their unique needs and preferences
- Facilitate interdisciplinary team meetings to discuss resident care, ensuring collaboration among nursing, medical, and supportive staff
- Regularly review and update care plans based on resident assessments, family feedback, and observed changes in condition
- Regularly review and ensure that staff are well aware and knowledgeable about residents in their care
- Oversee the maintenance of accurate and appropriate records for each resident and prospective resident of the community, for each employee and other administrative activities, and maintain confidentiality of said files at all times
- Assist as needed in the coordination of move-in with respective residents. Periodically review each resident's status and ability to perform under the terms and conditions of the Residency Agreement
- Support oversight of the process of determining each resident's suitability and placement for transfer to an environment appropriate for their mental and physical condition. Direct and advise as needed the process for notifying staff of new residents’ needs and ensuring the apartment is move-in ready
- Attend and engage in daily department leader meetings to focus on resident condition, admissions, discharges, resident/family concerns, life safety issues, employee injuries, and resident accident/incidents
- Ensure compliance with all state and federal regulations related to resident care, including Title 22 of the California Code of Regulations and the Health Insurance Portability and Accountability Act (HIPAA)
- Stay informed of changes in laws and regulations that affect facility operations and resident care
- Participate in inspections and audits by state licensing agencies, responding promptly to any regulatory compliance issues
- Serve as the primary point of contact for residents and their families, addressing concerns and facilitating communication regarding care and services
- Organize and participate with relevant leadership and staff in family meetings to discuss care plans, changes in resident conditions, and available resources
- Develop and implement resident engagement programs that promote social interaction and cognitive stimulation
- Monitor and evaluate the quality of care provided by staff, using assessments and resident feedback to identify areas for improvement
- Implement quality improvement initiatives to enhance resident care and satisfaction levels
- Maintain accurate and timely documentation of resident care, staff performance, and compliance activities in accordance with California laws
- Routinely observe and report on the sanitation of the kitchen, quality of meals, quantity and appearance of service and care staff, cleanliness of resident apartment and satisfaction of residents
- Responsible for implementation and follow through of recommendations of the safety committee to provide a safe environment for residents, staff, and visitors
- Provide support and guidance to staff during emergency situations, including behavioral crises and medical emergencies
- Develop and implement protocols for handling emergencies related to resident care and safety with the Executive Director
- Foster relationships with local healthcare providers, community organizations, and service agencies who engage with the community to enhance resident care and community outreach
- Represent the facility at networking events and workshops when necessary, advocating for the needs of residents with Alzheimer's and dementia
- Assist in budget planning by providing suggestions and insights regarding staffing needs and resource allocation to meet resident care standards
- Support monitoring of expenditures related to caregiving services and supplies
- Perform any additional job duties set by the corporation while completing all assigned duties which may change from time to time according to resident needs, staffing levels, working circumstances, and corporate directives
Qualifications
- Bachelor’s degree in Nursing, Social Work, Gerontology, or related field preferred; certification as a Nursing Home Administrator (NHA) or Assisted Living Administrator (ALA) is a plus
- RCFE Certification
- Medication authority: certified Medication Aide (CMA) or documented competency/training if the RCC will administer medications (or follow facility policy/physician orders)
- Valid California RN licensure preferred; certified in Basic Life Support (BLS)
- Valid CPR/First Aid training
- Minimum of 3 years of experience in a caregiving or management role within a dementia/Alzheimer's facility
- Strong leadership and supervisory skills
- Excellent interpersonal and communication abilities
- Knowledge of California regulations regarding long-term care and dementia care
- Familiarity with electronic health record (EHR) systems and documentation standards
- Capacity to develop and implement training programsOther Requirements
- Good communication skills, verbal and written; English language skills adequate to allow communication with residents and staff, and to understand written and verbal instructions
