JOB SUMMARY

The PA or NP is responsible for providing relief from the symptoms of patient’s chronic progressive illnesses and preventing hospitalization through triage and coordination of care.  This position will serve the Tucson/Oro Valley area!

MAJOR JOB FUNCTIONS

  • Promote and practice the Sage Palliative philosophy and mission: To provide the absolute most compassionate, comprehensive care to people with chronic illness, pain or emotional duress in order to serve them through their individual journey with dignity, respect and integrity.
  • Working knowledge of the regulatory requirements per state licensure, federal guidelines.
  • Provide on-call support and work closely with triage team to prevent hospitalizations and provide patient support.
  • Collaborate with health care partners and referral sources to promote and sustain community relationships.
  • Develop treatment plans to provide symptom management.
  • Coordinate with patient’s care team and referral sources to improve patient care and prevent readmissions.
  • Arrange for comfort measures such as pain management, home health and spiritual care.
  • Work directly with social workers to assist in end-of- life discussions and hospice transitions, if necessary.
  • Document patient visits in a timely manner for billing and expedition of patient care.
  • Advocate for healthcare changes which have the potential to improve quality of life and address issues of grief of patients and families.
  • Work directly with Palliative Director and Productivity Specialist to produce necessary documentation CPT codes for patient care and billing.
  • Continuously seek better processes to maximize provider and administrative staff productivity.
  • Maintain proper certifications and licensing per state guidelines.
  • Proficient in accessing and utilizing the organization’s EMR. 

REQUIREMENTS

  • Master’s degree in nursing from an accredited program with current license to practice professional nursing in the state in good standing.
  • Board certified advanced nurse practitioner or physician assistant
  • Current DEA
  • CPR certification
  • Minimum of 2 years of health care experience in hospital, medical outpatient clinic, home health, hospice or specialty care setting.
  • Must be a licensed driver with an automobile that is in good working order and demonstrate ongoing insurance in accordance with state and or organization requirements.

SKILLS/ABILITIES/KNOWLEDGE

  • Self-directed and able to work with minimal supervision.
  • Ability to triage and manage own work with very little supervision.
  • Excellent observational, written and verbal communication skills.
  • Proficient customer-focused service skills, patient-centered, positive problem-solving advocate.
  • Understanding of the principles of pain/symptom management.
