Role Description
This is a full-time hybrid role for the Director of Case Management CDI, based in Lake Charles, LA, with the opportunity to work remotely on a limited basis. The Director of Case Management CDI will oversee the development, implementation, and management of case management and clinical documentation improvement (CDI) processes. Responsibilities include managing discharge planning, optimizing utilization management strategies, coordinating medical case management, ensuring compliance with Medicare requirements, and collaborating with cross-functional teams to meet the organization’s goals.
Qualifications
- Strong expertise in Discharge Planning and Utilization Management
- Proven experience in Medical Case Management and Case Management
- Comprehensive knowledge of Medicare processes and regulations
- Excellent leadership, problem-solving, and decision-making abilities
- Ability to manage cross-functional teams and implement process improvements
- Bachelor’s degree in Nursing, Healthcare Administration, or related field; Master’s degree preferred
- Registered Nurse (RN) license required; Case Management Certification (CCM) preferred
