Anticipated End Date: 2026-06-05 Position Title: Community Engagement Navigator-1 Job Description: Community Engagement Navigator Virtual: The candidate selected for this role must reside in the state of California. This role enables associates to work virtually full-time, except for required in-person training sessions, providing maximum flexibility and autonomy. This approach promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development. Alternate locations may be considered if candidates reside within a commuting distance from an office. The Community Engagement Navigator is responsible for engaging with members to remove barriers to Social Determinants of Health (SDOH) and acting as the communication liaison between the company and referral sources. How you will make an impact: Engage with members who have been identified as possessing significant SDOH barriers. Work with the member to establish goals and next steps for addressing SDOH. Support member in selecting provider or programs most appropriate for supporting their needs. Interface with HCM to ensure that the individualized plan is aligned with existing or needed case management efforts. Act as a liaison between referral sources and the health plan. Minimum Requirements BA/BS degree in Public Health, Social Work, Rehabilitation Counseling or related field and a minimum of 2 years experience with marketing, job development, job placement or social work; or any combination of education and experience which would provide an equivalent background. Preferred Skills, Capabilities and Experiences: Knowledge of Voc Rehab preferred Travel may be required.
