- Coordinate with all parties involved in the Maryland Disability Benefits Advocacy Project (MD DBAP) to support customers through the Social Security Disability Benefits application process.
- Serve as a liaison between disabled customers and the Social Security Administration, advocating on behalf of customers to help obtain disability benefits.
- Schedule and maintain case screenings, interviews, and follow-up activities with customers and other involved entities.
- Conduct face-to-face and phone interviews to gather information needed for Social Security Disability case screenings and applications.
- Explain the disability benefits process, required documentation, and project expectations to customers while encouraging ongoing communication.
- Review documentation and screening information to assess eligibility for Social Security Disability Benefits.
- Obtain signatures on required authorization and acknowledgment forms.
- Develop comprehensive lists of medical treatment sources and coordinate the collection of supporting medical records.
- Encourage customer participation in obtaining medical documentation from healthcare providers.
- Coordinate with customers, medical providers, Maryland Department of Social Services, Disability Determination Services, and the Social Security Administration to obtain and submit required records.
- Enter case information into the project’s proprietary database, including data entry, document scanning, sorting, and record maintenance.
- Review medical records for accuracy and ensure patient information is complete and correct.
- Use medical documentation to build, supplement, and maintain complete disability case files.
- Conduct ongoing follow-up with customers, medical providers, and government agencies to support successful claim processing.
- Maintain organized, accurate, and complete case records throughout the application lifecycle.
- Experience in case management, social services, disability advocacy, benefits coordination, healthcare administration, human services, or a related field.
- Strong interviewing, customer service, and communication skills.
- Ability to assess documentation and determine eligibility requirements.
- Experience coordinating with multiple stakeholders, including government agencies, healthcare providers, and customers.
- Strong organizational skills with the ability to manage multiple cases simultaneously.
- Accurate data entry and document management skills.
- Ability to review, organize, and maintain confidential records.
- Strong follow-up, problem-solving, and case coordination abilities.
- Ability to work independently while maintaining attention to detail.
- Proficiency with computer systems, databases, scanning, and electronic document management.
- Knowledge of Social Security Disability, SSI, public assistance programs, or benefits administration is preferred.
- Primary focus is supporting Maryland residents applying for Social Security Disability Benefits.
- Role involves frequent interaction with customers, healthcare providers, the Social Security Administration, Disability Determination Services, and the Maryland Department of Social Services.
- Daily responsibilities include interviewing, case documentation, medical record collection, data entry, and ongoing case follow-up.
