What are the responsibilities and job description for the Connected Care Specialist position at Electronic Caregiver?
As a Connected Care Specialist, you will engage eligible patients/customers through consultative, value-driven conversations to educate, guide, and enroll them into Chronic Care Management (CCM) and/or Remote Patient Monitoring (RPM) programs, using tools such as Virtual Caregiver Addison when applicable. This phone-based role requires accurate electronic documentation and collaboration with patients, providers, clinical staff, and internal teams to optimize care. • Meet enrollment performance expectations, with quotas determined by the size of the assigned lead list and the organization’s target enrollment rate; performance is measured against these established targets. • Achieve and maintain required quality metrics, including Customer-Critical scores of 95% or higher, Business Critical scores of 90% or higher, and Compliance-Critical scores of 99.5% or higher, while adhering to CMS and HIPAA regulations. To be a successful incumbent should be observant and detail oriented and possess an understanding of the business, the products and services. Work is performed under immediate supervision by a project lead or department head: however, the employee is expected to exercise good judgement and discretion within limits of established procedures.
