We are seeking compassionate and dedicated Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to support our mission of enhancing the well-being of our clients.
Spanish-speaking nurses are especially needed to better serve our diverse community.
Position Overview
RNs: Conduct supervisory visits, fulfill RN-specific responsibilities, and provide medication management for assigned clients.
LPNs: Assist primarily with medication management, ensuring clients stay on schedule and maintain independence at home.
Responsibilities
- Perform supervisory visits to evaluate care quality and support caregivers (RN role).
- Provide medication management services: set up pillboxes, verify accuracy, and educate clients/families.
- Develop and implement individualized care plans.
- Maintain accurate documentation of visits, medications, and care.
- Collaborate with clients, families, and care teams to adjust care as needed.
Requirements
- Active RN or LPN license in Michigan.
- Reliable vehicle with valid driver’s license and current auto insurance.
- Active cell phone for communication.
- Strong organizational skills and attention to detail.
- Excellent communication with clients, families, and care teams.
- Compassionate and committed to supporting older adults.
- Home health care experience preferred, but not required.
- Spanish-speaking skills highly valued.
