An ASAP RN conducts assessments and screenings for Home Care, CAE, and all other agency programs. Consults with agency staff as necessary and develops care plans as needed.
- Conducts clinical screenings to determine consumer’s level or care for personal care services, eligibility for certain Home Care programs, and screenings for Medicaid consumers for skilled nursing facilities or Adult Day Health programs.
- Case conference with Care Managers during assessment, screening or care planning and as needed for consumers.
- Provides consultation with agency staff as needed, such as Protective Services issues.
- Conducts on-site assessments and visits to skilled nursing facilities.
- Maintains tracking schedule for assessments and follow up contact with consumers.
- Maintain consumer records, ensuring compliance with Executive Office of Elder Affairs and Elder Services of Cape Cod and the Islands.
- Contact community agencies, vendors, hospitals, skilled nursing facilities, physicians, consumers, families, and others as needed.
- Attend assigned Team meetings weekly.
- Other duties as assigned.
- Ability to collaborate, take initiative, provide/ receive feedback.
- Excellent computer skills required.
- Ability to work independently and professionally.
- Ability to communicate and work with the elderly and community resources serving the elderly population.
- Valid MA driver’s license and car required
- Graduate from an accredited School of Nursing, BS preferred. Valid RN License.
- Minimum of one-year clinical experience in community health and/or long-term care
- Prolonged periods sitting at a desk and working on a computer.
- Must be able to lift up to 15 pounds.
- Travel required on a regular basis throughout services area including the Islands. Occasion off-cape travel.
