Social Work, Case Management(Full-Time)Position Summary
The Social Worker/Case Manager is responsible for supporting the dignity, independence, and well-being of older adults and individuals with disabilities who require assistance. This role involves assessing client needs, identifying service gaps, coordinating and monitoring support services, and advocating on behalf of clients to ensure they receive appropriate resources and care.
This position is with a nonprofit organization dedicated to enhancing the quality of life for vulnerable populations within the community.
Qualifications
- Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related field preferred, but not required.
- Candidates with a bachelor's degree in another discipline may be considered if they can demonstrate relevant experience and/or a strong interest in human services through previous employment, internships, volunteer work, or additional coursework.
- Valid driver's license required.
- Reliable transportation and willingness to travel throughout the service area required.
- Strong proficiency in Microsoft Office applications required.
- Experience using databases and case management systems preferred.

Pay: $23.59 - $24.62 per hour
