What are the responsibilities and job description for the LPN - In Home position at Elder's Journey?
We are seeking a dedicated Home Care Nurse to join our team and provide compassionate 1:1 care to a patient in their home. The ideal candidate will have a strong background in various medical procedures and a passion for delivering high-quality healthcare services.
Duties
- Provide skilled nursing care to patients in their homes
- Perform physical examinations and assessments
- Administer medications and treatments as prescribed
- Monitor patient vital signs and condition
- Educate patients and families on healthcare management
- Collaborate with physicians and other healthcare professionals to develop care plans
- Assist with wound care, IV therapy, and other medical procedures as needed
Qualifications
- Experience in infectious disease care, cardiac catheterization, acute care, OR, ICD-10, phlebotomy, and medical imaging
- Ability to provide comprehensive home community care
- Strong knowledge of hospital medicine practices
- Excellent communication and interpersonal skills
- Current nursing license and CPR certification
- Bachelor's degree in Nursing or related field preferred
- Previous experience in home health or hospice care is a plus
