For over 51 years, Elder Care Services has been fulfilling our mission to improve the quality of life for seniors and their caregivers in the Big Bend. Elder Care Services currently serves as the Lead Non-Profit Agency for in-home services in Gadsden, Leon, and Wakulla Counties.
Our comprehensive services provide community based, compassionate care for a broad spectrum of seniors. For active seniors, we provide an avenue to stay engaged in the community through volunteering placement. For families beginning the aging process, we are an advocate and trusted resource. For frail and homebound seniors, we continue to provide a variety of services that promote independence and safety while individuals age in place.
We are looking for a compassionate, detailed, and highly motivated Nutrition Case Manager to join our team. As our Nutrition Case Manager, you will have the opportunity to make a positive difference in the lives of seniors in the Big Bend area through our Meals on Wheels program. We will provide an hourly rate within the range of $17.50-$19.00 per hour (depending on education level and experience) . Qualifications and Education Requirements
- A bachelor’s degree in social work, sociology, psychology, nursing, gerontology or a related social services field is required; or four years of related job experience or any combination of education and related experience may be substituted for a bachelor’s degree upon approval of the Area Agency on Aging.
- Basic computer skills and knowledge is required.
- A reliable personal vehicle available and a good driving record, travel is required within in Leon County.
- Successful completion of Area Agency on Aging for North Florida Case Manager certification as made available by the Area Agency on Aging
- Employee must successful complete the 701B Assessment training within 10 business days of hire.
- Available to work hours beyond the usual workday of 8:00 a.m. to 5:00 p.m. when necessary
While performing the duties of this job, the employee will spend time in client homes and will be exposed to a variety of conditions and elements. The noise level is moderate to quiet.
Role and Responsibilities
- Follow prioritization procedure for enrolling new clients in Meals on Wheels program.
- Work closely with Clinical Director to determine when new clients should start services from waiting list.
- Complete initial client assessments to identify: Appropriateness for the MOW program, problems and immediate needs of the client, client’s strengths and ability to cope with his/her limitations, support available from caregivers and/or other resources, and needs for referrals to other programs.
- Generate information on new clients for entry in CIRTS and route assignment.
- Evaluate client needs and refer client for placement on appropriate waiting lists.
- Educate clients on nutritional programs available to them including Meals on Wheels, Congregate Meals and Frozen Meals.
- Conduct annual assessments on all MOW – C2 clients on caseload and determine if client still qualifies for MOW services and if any new gaps in services exist.
- Conduct periodic re-assessment interviews with persons receiving services to determine continued eligibility and increased need.
- Maintain client files with all required documentation.
- Follow up on clients who do not answer the door during Meals on Wheels delivery to ensure safety of client.
- Substitute for other Nutrition Program staff and Meals on Wheels volunteers, as needed.
- Provide case management to clients who may be at risk for social isolation
- Perform other duties as assigned by the Clinical Director and/or the President/CEO.
