Job Duties
- Assist the Social Worker in completing, filing, and maintaining psychosocial assessments, progress notes, and care plan documentation.
- Prepare and update resident charts and records in accordance with facility policies and regulatory standards.
- Organize and maintain departmental files, correspondence, and reports as delegated by the Social Services Director.
- Review and Issue Notice of Medicare Noncoverage and ABNs timely and efficiently and document accordingly
- Assist with meeting new residents and their family/representatives upon admission and gathering information
- Provide clerical support during admissions, transfers, and discharges, including collecting required documentation and updating records
- Assist with referrals and follow-up communications with community agencies under the direction of the Social Worker
- Gather and input information to support timely and accurate completion of MDS assessments under the direction of the Social Worker
- Utilize a variety of teaching methods including presentations, online learning modules, simulations, demonstrations, and hands-on training
