Responsibilities:
- Provide neurological evaluations and follow-up care for nursing home residents
- Diagnose and manage conditions such as stroke recovery, Parkinson’s disease, dementia, neuropathy, and other neurological disorders
- Collaborate with nursing home staff, families, and interdisciplinary teams to create comprehensive care plans
- Document all patient encounters and maintain compliance with healthcare regulations
- Educate patients, caregivers, and facility staff on neurological conditions and treatments
- Licensed Nurse Practitioner (NP) in New York State
- Board Certification in Neurology or experience in neurology preferred
- Minimum 1-2 years of NP experience (geriatrics or neurology a plus)
- Experience in long-term care or nursing home settings preferred
- Strong clinical judgment and the ability to work independently
- Excellent communication and documentation skills
