Reports To: Director of Nursing, Assistant Director of Nursing, or MDS/Rehabilitation Director.

FLSA Status: Non-Exempt

This position plays a vital role in helping residents maintain independence, prevent functional decline, and achieve the highest practicable level of well-being in accordance with the facility's mission and regulatory requirements

Position Summary

The Restorative LPN is responsible for coordinating, implementing, and monitoring restorative nursing programs that promote residents' highest level of physical, mental, and psychosocial functioning. Working collaboratively with nursing staff, rehabilitation therapists, physicians, and interdisciplinary team members, the Restorative LPN develops individualized restorative care plans that support residents in maintaining or improving their independence and quality of life.

Essential Duties and Responsibilities

Resident Care and Program Management

• Assess residents for restorative nursing needs and identify opportunities to maximize functional abilities.
• Develop, implement, and update individualized restorative nursing programs in collaboration with the interdisciplinary care team.
• Coordinate restorative services including:
  • Range of motion (ROM) exercises
  • Ambulation and mobility programs
  • Bed mobility and transfer training
  • Splint and brace management
  • Self-care and activities of daily living (ADL) programs
  • Strengthening and endurance activities
  • Bowel and bladder retraining programs
  • Positioning and contracture prevention programs
• Monitor resident progress and modify programs as needed to achieve optimal outcomes.
• Ensure restorative services are delivered consistently and according to physician orders, care plans, and facility policies.

Documentation and Compliance

• Maintain accurate and timely documentation of restorative nursing services provided.
• Complete assessments, progress notes, restorative flow sheets, and required clinical documentation.
• Ensure compliance with federal, state, and local regulations, including Medicare and Medicaid requirements.
• Assist with MDS documentation and support care plan development related to restorative nursing services.
• Participate in quality assurance and performance improvement activities.

Collaboration and Communication

• Communicate resident progress, concerns, and changes in condition to appropriate team members.
• Work closely with Physical, Occupational, and Speech Therapy departments to promote continuity of care.
• Participate in resident care conferences and interdisciplinary team meetings.
• Educate residents, families, and staff regarding restorative nursing goals and interventions.

Leadership and Staff Support

• Provide guidance, training, and oversight to Certified Nursing Assistants (CNAs) and Restorative Nursing Assistants assigned to restorative programs.
• Monitor the quality and effectiveness of restorative services provided by direct care staff.
• Promote a resident-centered approach that supports dignity, independence, and quality of life.

Qualifications

Education

• Graduate of an accredited School of Practical Nursing.

Licensure

• Current, unrestricted Licensed Practical Nurse (LPN) license in the state of employment.
• Current CPR certification required.

Experience

• Minimum one (1) year of nursing experience preferred.
• Experience in long-term care, skilled nursing, rehabilitation, restorative nursing, or geriatric care preferred.
• Knowledge of restorative nursing principles, MDS, care planning, and regulatory requirements preferred.

Knowledge, Skills, and Abilities

• Strong understanding of restorative nursing practices and rehabilitation principles.
• Ability to assess resident needs and develop effective restorative interventions.
• Knowledge of long-term care regulations and documentation requirements.
• Excellent communication, organizational, and interpersonal skills.
• Ability to supervise and educate support staff.
• Proficient computer and electronic health record (EHR) skills.
• Ability to prioritize and manage multiple responsibilities.

Physical Requirements

• Ability to stand, walk, bend, lift, push, and pull during the course of work.
• Ability to assist residents with transfers, ambulation, and positioning as needed.
• Ability to lift up to 50 pounds with or without reasonable accommodation.
• Ability to respond appropriately in emergency situations.

Working Conditions

• Skilled nursing facility environment with frequent interaction with residents, families, staff, and healthcare professionals.
• Exposure to infectious diseases and bloodborne pathogens may occur; appropriate PPE and infection control practices are required.
