The Dental Health Home Coordinator is responsible for improving the population’s health, our patients’ experience of care; all while decreasing costs. As a liaison between the patient, El Rio Health’s Medical and Dental service lines, other medical entities, HMO’s, and inpatient facilities, the Dental Health Home Coordinator will ensure that the coordination of health care services, education, and case management, align with the patient’s health care needs with special focus on oral health treatment coordination. Additionally, the Dental Health Home Coordinator will collaborate with patients, medical and dental treatment teams, Health care providers, and other community services to manage population health data of the assigned patient panel, and reduce or eliminate care gaps to improve health outcomes. The Dental Health Home Coordinator will provide education to the patient that is pertinent to the patient’s condition and that empower the patient to make good health decisions. The primary goal of the El Rio Health Dental Health Home Coordinator is to support El Rio’s Mission of providing comprehensive, quality health care that is affordable and accessible to all who may have healthcare needs, by successfully performing the primary essential functions. This position is classified as a safety-sensitive position where the employee in this role has the responsibility for their own safety, other people’s safety, and may not be under the influence of substances that may prevent them from making coherent decisions, impair their judgement, or compromise the safety of any persons or property.   Minimum Education and Experience: Associate of Arts degree in Health Care, Human Services, or related field from an accredited college; Three (3)  years’ experience with direct patient contact either in a dental clinic or health care setting  Three (3) years’ experience working with Electronic Health Records (EHR) Required Licenses, Certifications, and Registrations: Certificate as a Dental Assistant or a Medical Assistant from an accredited program; Current cardiopulmonary resuscitation (CPR) certification issued by an approved certifying body; Employees in this position are required to have reliable transportation that can meet any operational; reassignments of the organization during the workday.  If an employee is driving during work hours, the employee is required to possess a valid driver’s license and must be in compliance with Arizona vehicle insurance requirements Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.   El Rio Health does not discriminate on the basis of race, color, religion, national origin, sex, sexual orientation, gender identity, age 40 and over, disability, genetic information, military status, ancestry, marital status, familial status, or any other status protected by law or regulation.
