Education and Experience: A Bachelor's degree or higher in Social Work, Behavioral Sciences, Health Education, Community Health Education, Public Health Education, School Health Education, Behavioral Sciences or a related field and two (2) years of experience in the HIV/AIDS field, providing health education in a public health, clinic and/or school setting or direct contact experience with the public, patient or provider in a communicable disease control, epidemiology or preventive health program.

Licenses and Certificates: Texas Class "C" Driver's License or equivalent from another state.

General Purpose

Under general supervision, plan, implement and assess an HIV client-centered program.

Typical Duties

Plan, implement and asses an HIV client-centered program. Involves: Implement an individual service plan to persons living with HIV or at risk for HIV/AIDS to include family members. Provide direct support services to client through one-to one contacts. Provide case management activities to include follow-up, service plan review, re-evaluations and revision of individual service plan. Conduct comprehensive assessments, client advocacy, support in short and long-term goals and conduct home visits as necessary. Teach, model and practice goal setting, problem solving and assertive communication. Conduct HIV testing, outreach, counseling to include venipuncture and finger sticks while working with the other programs within the HIV Prevention grant as needed. Educate individuals and groups on the epidemiology of HIV, modes of transmission and prevention. Maintain professional and ethical multicultural settings, including the ability to maintain confidentiality and privacy of persons, documents, data and communications. Stay current on community resources and provide appropriate referrals and information. Elicit information through intake screening interview and provide counseling. Initiate appropriate individual plans and referrals. Develop individual service plan for each client that support clients in short and long-term goals. Compile data to prepare records and reports.

Note: This is an unclassified contract position. Grant Funded.

Note: This position is working with the HOPWA program. Standard work hours will vary based on community demand and may include times outside the traditional Monday – Friday, 8:00 a.m. to 5:00 p.m. schedule. These positions may require working evenings and weekends as needed.
