Job Summary
This position will deliver compassionate, professional, and ethical supplemental services to program clients. LVN collaborates and coordinates effectively with others in the multidisciplinary team. The primary goal is to contribute to the provision of quality nursing care and achieve positive client outcomes through skilled techniques and expertise in client care.
Essential Duties And Responsibilities
- Assist the Program Director, Medical Director, psychiatrist and nurse practitioners with preparation of psychiatry clinic and daily tasks.
- Conduct assessments, medication authorizations, refills and address medication questions.
- Collect and document clinical data on clients such as assessments requested by providers. Examples include the PHQ-9, Vanderbilt, Screen for Child Anxiety Related Disorders (SCARED), and other tools implemented throughout services.
- Work with Quality Assurance team on running data, identifying trends, and implementing strategies for improved quality services.
- Conduct outreach via phone calls to high-risk clients to include clients on anti-depressants and with suicidal ideations/plan/intentions.
- Assist in scheduling clients for psychiatry related services.
- Apply considerable knowledge of client care, treatment, and special procedures related to practical nursing.
- Maintain thorough knowledge of medications, including reactions and dosage requirements.
- Maintain program specific databases containing client medication and assessment tools data.
- Report all reportable communicable diseases to the El Paso County Health Department following state and federal guidelines.
- Manage and maintain knowledge of all medical equipment used in client care.
- Assist in monitoring orders requested and received by providers through portals such as Quest Diagnostics.
- Provide care to clients with varying levels of mental health disorders.
- Monitor clients' treatment in conjunction with the psychiatry team.
- Educate clients and their families on treatment options and services available.
- Work closely with a multidisciplinary team, including psychiatrists and therapists and care coordinators, to develop individualized care plans.

Required/Preferred Education And Experience
- Graduate from a regionally accredited nursing school with LVN licensure in the State of Texas.
- One year experience collaborating with individuals who are diagnosed with a mental health disorder.
- Pursue continuous education and in-service training to expand knowledge of various age groups, treatment, and best practices.
- Any combination of education and experience that can be demonstrated to be applicable to the duties listed in the job description.
