Job Summary
The Care Navigator plays a vital role in supporting the intake and admissions process by serving as the first point of contact for individuals seeking services. The Care Navigator engages with clients, families, and referral sources in a professional and compassionate manner, providing clear and accurate information about available services and programs.
Essential Duties And Responsibilities
- Serve as the client liaison for all incoming intake calls, walk-ins, and referrals to the designated clinic, answering all calls within established guidelines in a welcoming, energetic manner.
- Initiates client pre-admission assessment and screening; Explains all services and levels of care offered in a concise and understandable manners to potential clients/family members.
- Screens and assessments for criteria that indicate participant is best served by another provider/entity/organization. Make referrals to outside agencies as appropriate, including for crisis services if indicated.
- Maintains working knowledge of community resources as well as organizational structure, current services and programming available within Center for effective referral for clients.
- Enter all admission and screening information in EHR in timely fashion; schedules initial intake appointment utilizing EHR for screened clients.
- Facilitates seamless client intake, monitors client no-shows, insurance, and qualifying documentation completion.
- Consult with Office Supervisor when indicated for referral partnerships, challenges in matching clients with available funding sources, and/or complex clinical presentation.
- Assist with all data and reporting functions related to this position. Tasks may include but are not limited to data entry, aggregation, and reporting.
- Assist with patient communication and workflow automation system for assigned clinic, with guidance and approval of Office Supervisor.
- Bachelor's degree in marketing, Business, Communication, Public Relations, or related field required, master's degree preferred.
- At least three years of experience in a related field and nonprofit sector.
Salary : $45,000
