Job Summary
Join our small, local home health therapy team as a PRN or part time Physical Therapist specializing in home health services. In this vital role, you will deliver personalized rehabilitation care directly to patients in their homes, supervise PTA’s, promote patient recovery, mobility, and independence. Your expertise will help improve quality of life for diverse patient populations, including geriatric, pediatric, neurological, and orthopedic cases. This position offers flexibility to work on an as-needed basis while making a meaningful impact on patient outcomes through compassionate, expert care.
Duties
- Conduct comprehensive assessments of patients’ physical function and develop tailored treatment plans aligned with clinical best practices.
- Implement therapeutic interventions such as manual therapy, trigger point therapy, and exercise programs to facilitate patient rehabilitation.
- Collaborate closely with healthcare professionals including physicians, nurses, and occupational therapists to coordinate care and discharge planning.
- Manage clinical documentation in accordance with healthcare standards, including detailed progress notes and treatment reports within electronic health records (EHR) systems.
- Educate patients and their families on home exercise programs, safety measures, and community resources to support ongoing recovery.
- Monitor patient progress regularly, adjusting treatment plans based on physiological responses and evolving needs.
- Ensure compliance with all home health regulations, safety protocols, and documentation standards while providing exceptional patient care.
Qualifications
- Valid state licensure as a licensed Physical Therapist with practicing experience in outpatient clinics or inpatient hospital settings.
- Demonstrated expertise in physiotherapy techniques across orthopedics, neurology, pediatrics, geriatrics, and outpatient rehabilitation.
- Strong knowledge of anatomy, physiology, medical terminology, and clinical documentation standards.
- Ability to collaborate effectively with multidisciplinary healthcare teams for comprehensive discharge planning and patient management.
- Excellent client communication skills to educate patients about treatment plans and motivate adherence to therapy programs.
- Prior outpatient or inpatient hospital experience is preferred; acute care experience is highly valued.
