Job Overview
We're seeking a Community & Case Management Partnerships Manager, to help expand access to medically tailored meals for seniors and vulnerable populations across Indiana.
This is an ideal opportunity for someone with experience in community outreach, provider relations, case management partnerships, Medicaid, healthcare business development, population health, home health, or managed care.
What You'll Do
- Build relationships with Case Management Organizations (CMOs), Medicaid partners, Area Agencies on Aging (AAAs), senior living communities, and community-based organizations
- Grow referral partnerships and increase meal utilization
- Conduct in-services, educational presentations, and community outreach
- Represent the organization at networking events, health fairs, and conferences
- Manage a referral pipeline and identify opportunities for growth throughout Indiana
What We're Looking For
- 3 years of experience in healthcare outreach, provider relations, community engagement, account management, case management, managed care, or business development
- Experience with Medicaid, LTSS, HCBS Waivers, Area Agencies on Aging, senior services, or community healthcare preferred
- Strong presentation, relationship-building, and communication skills
- Self-starter who thrives in a field-based, autonomous role
- Ability to travel throughout Indiana (60–75%)
Compensation & Benefits
- $85,000–$100,000 base salary
- 10–15% annual performance incentive
- Mileage reimbursement or vehicle allowance
